Claim for Damages forms and instructions are available in the City Finance Department, Attorney's Office, or you may use the Claim for Damages form provided on this web site. PLEASE MAKE COPIES FOR YOUR RECORDS PRIOR TO SUBMITTAL TO THE CITY.

Once the Claim Information has been received it will be forwarded to the City's insurance pool, Washington Cities Insurance Authority (WCIA). WCIA will proceed with an investigation of the claim and respond to the claimant directly regardless of whether the claim has been accepted or denied.Attorney
